WASHINGTON — The Department of Health and Human Services announced Friday a sweeping new initiative requiring all Americans to submit their daily step counts to a federal database, with those falling below 8,000 steps per day subject to mandatory “Movement Counseling Sessions” beginning in January 2027.
The National Mobility Accountability Act, passed quietly as an amendment to the Agricultural Appropriations Bill last month, establishes the Federal Step Registry and empowers HHS to monitor physical activity levels “in the interest of reducing national healthcare expenditures and promoting collective wellness.”
Secretary of Health and Human Services Dr. Patricia Mendez called the program “a common-sense approach to preventive medicine.” She explained that fitness trackers, smartphones, and smart home devices will automatically report step data to the registry through a new API that major tech companies have agreed to implement by September.
Americans who fail to meet the 8,000-step minimum for three consecutive weeks will receive a notice requiring them to attend a four-hour Movement Counseling Session at their nearest federally designated Wellness Hub. The sessions, which cannot be completed remotely, will include instruction on “productive ambulation,” “sedentary behavior reduction,” and “patriotic fitness planning.”
Repeat offenders may face escalating consequences, including enrollment in the Monitored Mobility Program, which requires participants to wear government-issued ankle monitors that vibrate every 30 minutes if insufficient movement is detected. According to internal HHS documents, the program will be funded by a new $12 monthly “Wellness Infrastructure Fee” added to all health insurance premiums.
Exemptions will be available for individuals with documented mobility limitations, though applicants must provide medical certification and renew their exemption status quarterly. Standing desks and treadmill desks will qualify for new federal tax credits of up to $400.
The first Wellness Hubs are expected to open in 47 major metropolitan areas by December, with rural areas served by mobile counseling units beginning in spring 2027.
